STEP 1 : COMPLETE THE REQUIRED NUMBER OF COPIES OF THE APPLICATION 

If you fill in the application manually: fill in 2 copies of the application in an identical way, or fill in the application 1 time and before you sign it anywhere – photocopy it 1 time to get 2 identical copies of the application. 

If you are completing the application on a computer: print out 2 copies of the completed application.

*If you are attaching Attachment No. 1 – ask your employer to complete it in 1 copy. Ensure that the employer’s signature is legible.

*If you are attaching Attachment No. 2, please print it out or complete it manually in 1 copy.

STEP 2: SIGNATURE

Sign in two places on each copy of the application:

– in the box in section D. (signature model – p. 7) – sign here in the same way you normally sign on documents, but ensure that your signature fits in the box.

– in part E. (DECLARATION – p. 8) – here your signature must be legible so that your name is clearly visible.

If you are attaching Attachment No. 1 – it should be signed by your employer.  Ensure that your employer’s signature is legible.

If you are attaching Attachment No. 2 – sign it personally. Your signature must be legible so that your name is clearly visible.

STEP 3: ATTACH THE REQUIRED DOCUMENTS

Attach all required documents to your application (see list of attachments >>> LINK). You may attach copies of documents, although it is necessary to present the originals for inspection during your visit to the office, if they are required. You do not have to present the originals if you submit notarized copies of the documents.

STEP 4: PAY THE STAMP DUTY

You must pay stamp duty for each application:

– for a temporary residence and work permit: PLN 440

– for every other temporary residence permit: PLN 340

Fees can be paid at the cash desk of the voivodship office, at the post office or by bank transfer to the account of the relevant voivodship office. The account details can be found on the website of the department handling residence permits of your relevant office. The fee must be paid before the application for a permit is submitted and the payment receipt must be enclosed with the application.

If you receive a positive decision before you collect your residence card, you will still be required to pay for the document itself – the residence card. The fee for issuing a residence card for an adult is PLN 50.

STEP 5: SUBMIT YOUR APPLICATION TO THE OFFICE

Submit all documents together with the attachments to the competent voivodship office, i.e. the one where you live. The address details can be found here: https://www.gov.pl/web/mswia/urzedy-wojewodzkie. In most offices, the submission of your application can be arranged by telephone or via an online calendar. You can also send the application with attachments by post, but then you will be asked to come to the office in person to provide fingerprints and original documents, if required.

STEP 6: PROVIDE FINGERPRINTS

If you apply personally during an appointment, you will be asked to provide your fingerprints. If you send your application by post, the authorities will ask you to come to the office in person to provide fingerprints. You will not be granted a residence permit without this formality.

If you are completing the application on a computer: print 2 copies of the completed application. 

*If your child is less than 13 years old, complete or print only 1 copy of the application.

STEP 2: SIGNATURE

A parent or legal representative should sign each copy of the application on behalf of the child. It should be signed in three places: on pages 7, 8 and 10, wherever there is space for a signature – in these places the signature must be legible, i.e. the name and surname should be legible. 

* Box in section E. (MODEL SIGNATURE) is left blank.

STEP 3: ATTACH THE REQUIRED DOCUMENTS 

Attach all required documents to your application (see list of enclosures >> LINK). You may attach copies of the documents, but you will need to show the originals for inspection during your visit to the office, if they are required. There is no need to show originals if you submit notarized copies of documents.

* Please note that you must include photographs of your child with your application regardless of their age.

STEP 4: PAY THE STAMP DUTY

For each application there is a stamp duty to be paid: PLN 340

The account details can be found on the website of the department handling residence permits of your relevant office. The fee must be paid before the application for a permit is submitted and the payment receipt must be enclosed with the application. 

If you receive a positive decision before you collect your residence card, you will still be required to pay for the document itself – the residence card. The fee for issuing a residence card for a child is PLN 25.

STEP 6: COME WITH YOUR CHILD TO THE OFFICE FOR FINGERPRINTING

If your child is already 6 years old, he or she should come to the office to be fingerprinted. If you submit the application in person, during an appointment, your child will be asked to provide fingerprints on that occasion. If you send the application by post, the office will ask your child to appear in person at the office to provide fingerprints.

Children under the age of 6 do not submit fingerprints and do not have to appear in the office.
