Permit for permanent residence

Each application must be accompanied by a set of documents that are required by law. If you do not have all the documents on the day of your application, it is possible to submit them later. Learn which documents to attach to your application for a permanent residence permit, depending on the basis of your application.

My parent holds a permanent residence permit or a residence permit for a long-term EU resident

– 2 copies of the application for a temporary residence permit (2 originals or 1 original plus copy with original signatures)

– 4 up-to-date colour photographs, 35 x 45 mm in size

– 2 copies of a valid travel document (all written pages – stamps, entries). If you do not possess a travel document, in particularly justified cases you may submit copies of another identity document

– confirmation of payment of the stamp duty to the office’s account

– documents confirming the required uninterrupted residence in Poland (passport pages, residence cards, decisions)

– documents confirming kinship with a foreigner who has a permanent residence permit/long-term resident of the EU: a complete copy of birth certificate registered at the Polish Register Office or a foreign birth certificate legalised or provided with an apostille clause and its translation into Polish made by a Polish sworn translator

– documents proving that the parent holds a permanent residence permit or an EU long-term resident permit

I am the child of a Polish citizen and I remain under his/her parental authority

– 2 copies of the application for a temporary residence permit (2 originals or 1 original plus copy with original signatures)

– 4 up-to-date colour photographs, 35 x 45 mm in size

– 2 copies of a valid travel document (all written pages – stamps, entries). If you do not possess a travel document, in particularly justified cases you may submit copies of another identity document

– confirmation of payment of the stamp duty to the office’s account

– documents confirming kinship with a Polish citizen: full copy of birth certificate registered at the Polish Registry Office or foreign birth certificate legalized or provided with an apostille clause and its translation into Polish made by a Polish sworn translator

– the parents’ marriage certificate or certificate of recognition of the child by a Polish citizen if the child was born before the parents’ marriage

– a copy of the identity card of the parent who is a Polish citizen

I am the spouse of a Polish citizen

– 2 copies of the application for a temporary residence permit (2 originals or 1 original plus copy with original signatures)

– 4 up-to-date colour photographs, 35 x 45 mm in size

– 2 copies of a valid travel document (all written pages – stamps, entries). If you do not possess a travel document, in particularly justified cases you may submit copies of another identity document

– confirmation of payment of the stamp duty to the office’s account

– documents confirming the required uninterrupted residence in Poland (passport pages, residence cards, decisions)

– a recent copy of the marriage certificate (issued not earlier than 3 months before submitting the application)

– if there are joint children, copies of birth certificates

– documents proving the existence of a joint household, e.g.: joint bills, payments for jointly occupied premises, joint bank account or authorizations to spouse’s bank accounts, joint correspondence, joint photographs from different periods, e.g. family celebrations, trips, etc.

I am of Polish origin

– 2 copies of the application for a temporary residence permit (2 originals or 1 original plus copy with original signatures)

– 4 up-to-date colour photographs, 35 x 45 mm in size

– 2 copies of a valid travel document (all written pages – stamps, entries). If you do not possess a travel document, in particularly justified cases you may submit copies of another identity document

– confirmation of payment of the stamp duty to the office’s account

– documents confirming the required uninterrupted residence in Poland (passport pages, residence cards, decisions)

– original documents proving Polish origin of one of the parents, grandparents or two great-grandparents

– documents proving your relationship with a person of Polish descent

– documents indicating the intention to settle in Poland permanently (documents from previous stays in Poland, lease or ownership agreements of premises in Poland, documents confirming that you are working or running a business in Poland, documents confirming that you have completed studies here or are still studying, evidence of permanent residence in Poland of members of your family with documents confirming their relationship). NOTE: do not be concerned if you do not have such documents – your statement (made in the application memorandum) that you intend to settle in Poland permanently should be sufficient for the office.

I have a valid Pole’s Card (Karta Polaka)

– 2 copies of the application for a temporary residence permit (2 originals or 1 original plus copy with original signatures)

– 4 up-to-date colour photographs, 35 x 45 mm in size

– 2 copies of a valid travel document (all written pages – stamps, entries). If you do not possess a travel document, in particularly justified cases you may submit copies of another identity document

– confirmation of payment of the stamp duty to the office’s account

– documents confirming the required uninterrupted stay in Poland (passport pages, residence cards, decisions)

– photocopy of a valid Pole’s Card (Karta Polaka)

– documents indicating the intention to settle in Poland permanently (documents from previous stays in Poland, lease or ownership agreements of premises in Poland, documents confirming that you are working or running a business in Poland, documents confirming that you have completed studies here or are still studying, evidence of permanent residence in Poland of members of your family with documents confirming their relationship).  NOTE: do not be concerned if you do not have such documents – your statement (made in the application memorandum) that you intend to settle in Poland permanently should be sufficient for the office.
