STEP 1 : COMPILE THE REQUIRED NUMBER OF COPIES OF THE APPLICATION

If you fill in the application manually: fill in 3 copies of the application in an identical way or fill in the application 1 time and before you sign it anywhere – photocopy it 2 times to get 3 identical copies of the application.

If you are completing the application on a computer: print out 3 copies of the completed application.

STEP 2: SIGNATURE 

Sign in four places on each copy of the application:

– in the box in section E. (MODEL SIGNATURE) – sign here in the same manner as you normally sign on documents, but make sure your signature fits in the box.

– on pages 7, 8 and 9, wherever there is space for your signature – your signature must be legible in these places so that your name is clearly visible.

STEP 3: ATTACH THE REQUIRED DOCUMENTS

Attach all required documents to your application (see list of attachments >>>). You may attach copies of documents, but you will have to present the originals for inspection during your visit to the office, if they are required. You do not have to present the originals if you submit notarized copies of the documents.

STEP 4: PAY THE STAMP DUTY

For each application you have to pay stamp duty: PLN 640.

Fees can be paid at the cash desk of the relevant voivodship office, at the post office or by bank transfer to the account of the relevant voivodship office. The account details can be found on the website of the department handling residence permits of your relevant office. The fee must be paid before the application for a permit is submitted and the payment receipt must be enclosed with the application.

If you receive a positive decision before you collect your residence card, you will still have to pay for the document itself – the residence card. The fee for issuing a residence card for an adult is PLN 50.

STEP 5: SUBMIT YOUR APPLICATION TO THE OFFICE

Submit all documents together with the attachments to the competent voivodship office, i.e. the one in which area you live. Here you will find the address details: https://www.gov.pl/web/mswia/urzedy-wojewodzkie. In most offices, you can make an appointment to submit an application by telephone or via an online calendar. You can also send the application with enclosures by post, but then you will be asked to appear in person at the office to provide fingerprints and original documents.

STEP 6: PROVIDE FINGERPRINTS

If you apply in person during an appointment, you will be asked to provide your fingerprints. If you send your application by post, the authorities will ask you to appear in person to provide fingerprints. You will not be granted a residence permit without this formality.

2. On behalf of the minor child

STEP 1 : COMPILE THE REQUIRED NUMBER OF COPIES OF THE APPLICATION

If you fill in the application manually: fill in 2 copies of the application in an identical way, or fill in the application 1 time and before you sign it anywhere – photocopy it 1 time to get 2 identical copies of the application. 

If you are completing the application on a computer: print 2 copies of each application. 

*If your child is less than 13, complete or print only 1 copy of the application.

STEP 2: SIGNATURE

A parent or legal representative should sign each copy of the application on behalf of the child. It should be signed in three places: on pages 7, 8 and 10, wherever there is space for a signature – in these places the signature must be legible, i.e. the name and surname should be readable.

* Box in section E. (MODEL SIGNATURE) is left blank.

STEP 6: APPEAR TO THE OFFICE WITH YOUR CHILD FOR FINGERPRINTING

If your child is already 6 years old, he or she should appear in the office for fingerprinting. If you submit the application in person, during an appointment, your child will be asked to provide fingerprints on that occasion. If you send the application by post, the office will ask your child to appear in person at the office to provide fingerprints.

Children under the age of 6 do not submit fingerprints and do not have to appear in the office.
